Multiple reports confirm that actress Demi Moore has checked into a Utah rehab facility following her hospitalization late last month.

E! News was the first to report that Moore had checked into the Cirque Lodge addiction center, an extremely posh facility based in Sundance, Utah. E! News’ source adds that Moore has been in the facility a week after she was rushed to the hospital on Jan. 23 after suffering from seizure-like symptoms. "She's on total lockdown and only talking to a small group of people,” the source told the site, adding that Moore is being treated for an eating disorder as well as addiction issues.

A separate source confirmed to People that the 49-year-old Margin Call actress is in a treatment facility and not at her Los Angeles home.

A representative for Moore has not commented on E! News’ report. When Access Hollywood tried to get a statement, her representative said there was no comment beyond the original Jan. 24 statement. At the time, her representative only said, “Because of the stresses in her life right now, Demi has chosen to seek professional assistance to treat her exhaustion and improve her overall health...She looks forward to getting well and is grateful for the support of her family and friends.”

The 911 call that the Los Angeles Fire Department released revealed that Moore was in fact “convulsing” and inhaled something “similar to incense,” according to People. However, the name of what that substance was had been redacted. Moore, who spent time in rehab in the ‘80s, split from her husband, Ashton Kutcher, in November. Prior to Saturday’s report, E! News was told by a source that Moore was undergoing “spiritual counseling.”

E! News notes that the Cirque Lodge has catered to numerous celebrities in the past, including Lindsay Lohan, Mary-Kate Olsen and Eva Mendes.
