The episode starts with Sam and Dean driving and discussing what to do about the fallen angels. Dean expresses a fear for Sam; fear that he’s dying which then leads up to Sam lying in the hospital bed unconscious. The doctor comes in to tell Dean that his brother is dying and that “it’s in God’s hands now.” This, of course, angers Dean because of the ways God has disappointed him throughout the show’s episodes. Dean tries praying to Cas in a chapel, telling him he’s not mad about the angels falling but Cas does not answer. Dean then calls out to all angels asking for help. We flash to multiple angels hearing Dean asking for them to come help, and many of them head towards the hospital.
While all this is happening the scene flashes back to the car ride inside Sam’s head. Even in his own mind he looks to Dean for answers and advice. Sam questions whether he should be alive since Dean has no plans to make that actually happen. Bobby appears in the backseat and they all argue about whether Sam should be alive. Dean argues that he’s in the front seat because Sam wants to fight. This causes Bobby to be put into the front seat of the car with the boys and then Sam and Bobby vanish from the car and reappear alone in the woods.
While the boys are struggling with Sam’s condition, Cas is alone walking on the side of the road obviously lost and confused. He almost gets hit by a car and experiences pain for the first time. The driver gives him a ride to a gas station where Cas meets a fellow angel.
Dean is visited by a grief counselor who offers no help to Dean when he suddenly remembers that he has “the King of hell in [his] trunk.” Dean goes to talk to Crowley but then is attacked by an angel who wants Dean to help him. The angel is looking for Cas but Dean denies knowing where he is. Another angel appears and a fight starts between the two. Dean stabs the angel who attacked him and the remaining angel passes out. Dean traps him in a ring of hell fire and learns his name is Ezekiel. He offers to help Dean.
Cas and the other angel, Hael, discuss the many opportunities they can have now that they are on Earth, such as going to the Grand Canyon. Cas calls Deans cell and explains to him that Metatron tricked him and how he took his grace. Dean tells Cas about Ezekiel who Cas calls a “good soldier.” When told about Sam, Cas wants to come to the hospital but Dean tells him not to come because angels are after him and to not trust any of the angels. Cas tries to get Hael to go on her own so he can go visit Sam, but when he turns to leave she knocks him unconscious with a piece of wood.
Bobby tells Sam to just die and that he’s already done enough good. Sam and Bobby are talking in the woods when Dean stabs and kills Bobby because he was the part of Sam that wanted him to die. Sam asks Dean what his plans is and Dean starts punching him and yelling that his plan is to fight. Sam remains adamant about wanting to die and Dean vanishes.
New angels begin arriving at the hospital and Dean puts up sigils to guard the room against them. Dean leaves Ezekiel to tend to Sam while he distracts the angels. Two new angels beat up Dean questioning him where Cas is. After many beatings, Dean says he will tell them where Cas is but instead uses an angel sigil to force them out of the hospital. Ezekiel says he can’t save his brother’s life and the only way he may be able to help is by possessing Sam. Dean says Sam will never agree to it and Ezekiel goes to leave. Dean stops him and Ezekiel shows him what is going on inside Sam’s head. The two debate whether or not Sam will say yes to Ezekiel possessing Sam.
Cas wakes up in the car with Hael, who has an angel sword laying in her lap. Hael makes her intentions of possessing Cas’ vessel clear. Cas sees his only way out is to crash the car. When he awakes from the crash he tells Hael he wants to help the angels but Hael tells him they want him dead and they don’t want his help. Hael tries to blackmail Cas into letting her possess him but Cas stabs her with the angel sword and kills her.
Sam walks into the house that materialized in the woods to meet Death. “I’ve been waiting for you,” Death tells him, wanting to personally reap Sam’s soul. Sam asks Death if he goes with him will he stay dead; he does not want to be brought back to life, which Death agrees too. Dean shows back up in Sam’s mind and tells him it’s not his time to die. “There ain’t no me if there ain’t no you,” he tells him. Dean convinces Sam to say yes to Dean’s help and Dean turns into Ezekiel.
Ezekiel’s vessel wakes up on the hospital bed and Sam and Dean are gone. They walk away from the hospital, with Ezekiel clearly possessing Sam’s body. They debate whether or not to tell Sam about Ezekiel possessing him and agree it’s best to keep it a secret.
The episode ends with Dean and Sam driving down a road when Sam wakes up with no recollection of anything that has happened after the angels fell. Meanwhile Cas is still adjusting to life as a human and ways of obtaining food and supplies.
