The episode begins with a man tied to a chair and being hung over a building under a large ‘GOTHAM CITY’ sign. The killer drops the rope and lets the man fall and eventually hang to death. Maroni and Oswald are celebrating the alleged death of Fish and Oswald informs him that he took over the club in the Maroni family name. Maroni gets a phone call from Fish…which he is very surprised to receive. Fish tells Maroni that Oswald has been working for Falcone the entire time. Maroni seems skeptical to believe her, but the expression on his face appears to question Oswald. Maroni suggests that Oswald accompany him on a road trip.

Edward is investigating the crime scene we saw earlier and Harvey and Captain Essen are discussing their theories. Edward says he found an incision on the victim’s abdomen and suspects that the killer was searching for something. Captain Essen warns Edward that he is forensics only and needs to stop trying to be the medical examiner.

Selina is at Jim’s apartment when he arrives looking for Barbara. He notices that someone has been there and finds Selina as she is trying to hide. Jim says he will look for somewhere safe for Selina to live but she drops a bomb on him when she informs him that she was lying about seeing anything the night of the Wayne Murders’.

Jim visits Bruce at Wayne Manor and they discuss if Selina was lying or not. Bruce believes Selina and wants Jim to give up the case since has not produced any solid results. Bruce releases Jim from his promise and assures him he will handle this case on his own. Jim tries to talk with him but Alfred tells Jim once Bruce has made up his mind, he is not changing it.

Maroni and Oswald arrive upstate at a cottage. They discuss several things, including how Maroni saves Oswald’s life. He seems to be offhandedly interrogating Oswald and Oswald is slowly realizing this. Maroni goes outside to get some air while Oswald looks very uncomfortable.

Harvey meets Ms. Mullens who was a sponsor of Adam, the man who was murdered the night before. She was his sponsor of a phobia support group. Adam was afraid of heights. Harvey suspects someone in the group could be the murdered. Harvey promises Ms. Mullens that he will find Adam’s killer. Jim says he has a lead on the chair Adam was found in. It came from an abandoned company in The Narrows.

We see a man tasered and kidnapped on the street by Adam’s killer. Meanwhile, Edward is in the morgue examining Adam’s body and is caught by the medical examiner and Captain Essen. Essen has no choice but to suspend Edward until further notice.

Harvey and Jim search the building while discussing the current women in their lives. They hear a scream and find the second victim tied up and a man with a pig mask on. They shoot and kill the man in the mask. He is also from the phobia support group and Jim wants Edward to examine the body. Essen informs him of Edward’s suspension and that they need to be careful since everyone is watching everything they do.

Fish is on a boat that is leaving Gotham. “I’ll be back soon enough” she says as she lays in bed. Edward informs Ms. Kringle of his suspension. She looks sincerely sad that he is gone.
In the cottage Oswald finds one of Maroni’s guns and sticks it in his pants. Oswald and Maroni proceed to have a serious conversation about trust. It eventually leads to Oswald pulling the gun on Maroni after he tells Oswald that Fish is alive and claims he has been working with Falcone the entire time. Oswald pulls the gun on Maroni and shoots him, but the gun is loaded with blanks. Maroni says he knows Oswald played him and knocks him out.

Jim and Dr. Thompkins are out on a date and Dr. Thompkins is very happy to see Jim. Jim wants information on a case but also says he is happy to see her. The date is cut short when Jim gets a call that the second phobia victim has woke up. Harvey and Ms. Mullens are leaving the phobia support group and Harvey appears very smitten with Ms. Mullens. The murderer walks next to Harvey. He asks Harvey if this is his first meeting. “I’m a regular” says the killer.

Maroni has Oswald at a junkyard preparing to kill him. Oswald begs for his life but Maroni is hearing none of it. He throws Oswald in the car and the dump truck slowly starts crushing the car. Oswald calls Maroni from the car still trying to offer a deal. He claims whoever controls Arkham will control Gotham. Oswald then calls the man who is crushing the car and promises Falcone will send pain his way if he dies. The man stops crushing the car and Oswald is able to escape, much to the dismay of Maroni.

Edward sneaks into the men’s locker room to break into someone’s locker room. Harvey is at the phobia support group and talks about his fear of dying alone. We then see the killer whose name is “Todd.” His fear is failure and he gets extremely emotional and leaves the room and Ms. Mullens goes to check on him. Harvey notices they have been gone for a long time and goes outside to see Todd putting Ms. Mullens in the back of this van and driving off.

The medical examiner opens his locker to find several severed limbs in his locker. Jim tells Essen that the man “Todd” was at the meeting and still took Mullens even though he knew a cop was there. Mullens has a fear of water and Harvey knows exactly where Todd will take her.

We find out Todd is actually Dr. Crane and has taken Ms. Mullens to the pool she nearly drowned at when she was a child. Dr. Crane tries to reason with his son what he is doing to these people. Crane throws her into the pool and it appears she is drowning as Jim and Harvey show up and open fire on Crane. Jim pursues Crane while Harvey rescues Mullens. Crane is able to elude Jim while Harvey is able to save Mullen’s life.

Oswald is passed out on the side of the road and is approached by a group of women from a church bus. Oswald catches a ride with them back to Gotham. Dr. Thompkins gives Jim the information from the first phobia victim and they discover that Dr. Crane was looking to find the adrenal gland that controls fear. Jim tells her the medical examiner job is open and they should discuss details over dinner. Dr. Thompkins makes Jim kiss her in front of everyone at GCPD.

Edward informs Ms. Kringle that he is no longer leaving. Edward still has hope. It appears Ms. Kringle might have some feelings for Edward. Fish is sleeping on the boat when she hears gunshots. A mercenary shoots the captain of the ship and goes after Fish.
