In episode 4 of season 3 on Botched, Dr. Paul Nassif and Dr. Terry Dubrow once again help three of their patients feel more confident in their appearances. During the hour-long episode, the duo help a man feel more confident in his own skin, a woman with botched breasts and a woman with star-shaped implants in her ribs.

Victor, a female impersonator and the first patient featured in the episode, spoke about his life growing up as a gay man as he found a passion for beauty pageants and won seven national pageants. After a face full of makeup and wig on his head, he became Victoria. To get the look he desired, his facial procedures included receiving silicon injections in his nose and had a nose reconstruction that was considered botched. Over time, he had begun developing granulomas, or nodules and had six removed from his nose.

After a life of living as Victoria, he said he wanted to have a surgery to have a more natural and healthy looking nose to feel comfortable going out in public as Victor.

During the consultation, Dr. Dubrow said there was likely poor blood flow in his nose which could leave a lot of issues, including dead tissues. Despite the risks of having dead tissue, Victor wanted to go all in by having Dr. Nassif fix his botched nose.

“I would rather die trying than live like this the rest of my life, ashamed of who I am,” Victor said.

To complete the procedure, Dr. Nassif took rib cartilage and fascia from his head to reconstruct his nose without disrupting the granulomas on his nose. Dr. Nassif found fibrous scar tissue, making it hard for him to cut through the tissue. Later, he extracted a black head on his nose and discovered he did not have a granuloma, but a huge zit. After he recovered from the surgery, Victor came back to check in with Dr. Nassif. The doctor discovered he had a small amount of dead tissue and would have to go back into surgery remove the dead tissue and suture the good skin together.

After a successful second surgery, Victor visited his friends at a drag show as Victor, instead of Victoria. He said Dr. Nassif had given him back confidence and made him feel like he didn't have to have a different persona.

Kim, the second patient in the Botched episode, said she decided to get breast implants because she had low self-esteem. She wanted a C-cup size implant but ended up as a 32 DDD-cup. After feeling top heavy and experiencing pain in her neck and shoulders, the implants ruptured and silicon filled into her lymph nodes. After having three surgeries to try and get the look she desired, she ultimately had to get the implants removed. Her breasts then became deformed and caved in, which she said made her feel disgusted. She said it caused a lot of intimacy issues with her husband and that her husband has only seen her breasts once since the implants were removed.

During the consultation, Dr. Nassif said the implants she had were too large for her and her body reacted to it, causing scar tissue and the rupture. When Kim changed into the medical gown, her husband said he had to turn around to avoid seeing her breasts. After Dr. Nassif heard the expression from Kim’s husband, he explained to him that he needs to be more understanding and more supportive of his wife.

To complete the surgery, Dr. Dubrow said he will elevate the soft tissue of her rib cage to assess if her breasts can handle another implant. Because her lungs were near the area, he had a fear he may have collapsed a lung.

“You puncture the lung and you get a whole new set of problems that you don’t want to see,” Dr. Dubrow said.

The lung did not collapse and Dr. Dubrow was able to place specialized C-cup implants. The last scene of Kim’s story on Botched showed her feeling happy about her new breasts and her husband saying he will be more supportive in the future and is happy his wife is confident again.

Ashlea, a body piercing lover and final patient of the Botched episode, got star-shaped implants to sit on top of her ribs. After a lot of pain from the procedure, the stars were not sitting even on her body and began to hurt. With the hopes of becoming a yoga instructor, Ashlea is hoping to get the stars removed after having them for five years.

After looking at the implants, Dr. Dubrow said they were deep and  understood why she was in a lot of pain. To take out the implants, Dr. Dubrow said he would make an incision in her rib, remove them and stitch her back up. With the help of Dr. Nassif, the Botched doctors began the procedure while Ashlea was put under local anesthesia.

After a successful surgery, she said she can freely move and do more of what she enjoys.
