On season 3, episode 9, of Botched, Dr. Terry Debrow and Dr. Paul Nassif meet with three new possible patients who believe they need help with their botched body parts.
Carmen, a 33-year-old, was born as a male. She knew at an early age that she wanted to be a woman. After graduating from high school, she moved to Atlanta and saved her money. She went to Mexico to get her nose and breasts done and was castrated. She said it is hard to breathe out of the nose she has and has to stick a straw through her nose so she can breathe.
During her consultation, Carmen tells her story and says she went to Atlanta and Florida to try and get her nose fixed. After having three surgeries, she said her health is more important than her vanity. Nassif said the previous doctors took a lot of tissue from one side and says her nose has really been botched. After looking at her ears, Nassif said there is just enough cartilage in her ear that he can use to rebuild her nose. She agrees to the surgery.
To prepare for surgery, Nassif goes over the procedure with Carmen. He says he plans to take as much skin, tissue and cartilage from the ears and fascia from her scalp as much as he can to help build her nose. While doing so, his team will take a piece of her rib to also help build her nose. Nassif has enough fascia, cartilage and rib to help reconstruct her nose. He said she has had a deviated septum and an extreme blockage in her airway. One of her previous surgeries caused her to have a scar on her left side that blocked her airway. After removing the scar, Nassif said he thinks it will be a good result.

During her check-in, Nassif says her nasal tip looks great and that she needs to be extra careful since it is her fifth surgery. Later, Carmen goes to lunch with her friends and sister to show off her new nose. Her sister says she can see Carmen is more confident and happier with her nose. Carmen says she can breathe now and is truly happy.
Maria was 19 years old when her mother was diagnosed with breast cancer. After being tested for a cancer gene, she found she does have the gene and has a huge chance of getting breast cancer. She decided to have a double mastectomy to lower her risk of cancer and had to wait for her breast pockets to be comfortable enough to get implants. Her doctor told her about nipple banking, the process of moving the nipples to her abdomen to keep them alive.
While meeting with the Botched doctors, she tells them about her story. Dubrow said less than 2 percent of surgeons use nipple banking because it is usually unsuccessful. Maria said she won’t talk to her daughters about getting tested for the specific gene until her breast problems are fixed. While examining her breasts, Dubrow said she has many problems. He suggests removing the implants and replacing them with new ones and using 3D tattooing to tattoo nipples on. She agrees to the surgery and says she is excited.
On her surgery day, Dubrow prepares her for surgery. He will take her implants out, examine her breasts, tighten her breast mounds, reinforce her pockets with mesh and then place new implants in.
During surgery, he sees the previous doctors put in a piece of cadaver skin in her breast pockets instead of mesh. He will have to remove some of the cadaver skin to help her. In the other breast, he sees the implant has ruptured. To give her extra volume on the top of her breast to prevent rippling, he harvested fat from her abdomen to help her breast pockets.
During her check-in, she says she is not in pain. Dubrow briefs her on what he found during her surgery and then takes a look at what her breasts look like. Later, she gets nipples tattooed on her breasts. After looking in the mirror, she begins to cry of happiness.
Star, a woman who grew up in the United Kingdom and as a Jehovah’s Witness wanted to get plastic surgery. After she had plastic surgery, her mother kicked her out of her house. She has had numerous injections of botox, lip fillers, two breast surgeries, nose job and numerous liposuctions. She is hoping the doctors can give her bigger breasts and a smaller nose. She says if the doctors can not help her, she will go to Poland.
During her consultation, she tells the Botched doctors that she wants to look as fake as possible. She says she had a terrible childhood and does not want to look like her mother. She said her nose is constantly dripping and feels like her breasts are too close together.
In the exam room, Dubrow said he has to make sure her goal matches their natural aesthetic. Nassif looks at her nose and says her middle nose is collapsed. Dubrow looks at her breasts and says she does not have a uni-boob, but only has minor rippling. If she got her breasts bigger, it would cause more rippling. The Botched doctors say there is nothing they can do for her since she does not want a bigger nose or smaller breasts.
