Charlie Sheen's ex-wife, Brooke Mueller, is being evaluated at a hospital after it was reported that she and the two children she shares with the actor were missing.

TMZ reported that on Tuesday, Nov. 15, Mueller showed up barefoot at a bar in Utah along with her 7-year-old twins, Bob and Max and their nanny. Sources said that she was exhibiting bizarre behavior and fighting with the kids nanny so someone at the establishment called police but they all left before officers arrived. Mueller was later seen trying to get a hotel room in the area.

By Wednesday, the Salt Lake City Police Department released a statement announcing that Mueller, her children and the nanny had all been found.

"On the morning of November 16, dispatchers with the Utah Department of Public Safety received a welfare check call from a woman claiming to be the mother of Brooke Mueller. Troopers with the Utah Highway Patrol responded to the welfare check call and located Mueller, her two children and a nanny on Interstate 15 near Nephi, Utah," the statement read. "The children are safe and will be released to family. Mueller was taken to an area hospital for evaluation. The State Bureau of Investigation is currently investigating the incident. We do not have any more information to release at this time."

A source told People that the incident occurred after “Brooke went off of her medications.”

Meanwhile Sheen was made aware of the incident and prior to his children being found said that he was “just trying to get my kids home safe."
