The New York City medical examiner officially confirmed on Friday that actor Philip Seymour Hoffman died earlier this month with a fatal mix ox cocaine, heroin and other drugs in his system.

TMZ reports that the official cause of death was listed as an “acute mixed drug intoxication. In addition to cocaine and heroin, the other drugs found in his system were benzodiazepines, an anti-anxiety drug; and amphetamines.

As the New York Daily News notes, the initial autopsy on the Oscar winning actor’s body was inconclusive, so the medical examiner had to do toxicology tests.

Hoffman died on Feb. 2 and was found in his West Village apartment with a heroin needle still in his arm. It had been reported that around 50 bags of heroin were found in his apartment. He was 46-years-old.

Hoffman won an Oscar for his portrayal of Truman Capote in 2005’s Capote and was most recently seen in The Hunger Games: Catching Fire. He had filmed a Showtime pilot called Happyish and completed the films A Most Wanted Man and God’s Pocket.
