Glen Campbell will release the final single of his career at the end of this month, providing his legion of fans with a heartbreaking and personal goodbye.

Campbell, 78, was diagnosed with Alzheimer’s disease back in 2011 and his health has been steadily declining since, sadly. In April, he was moved to an Alzheimer’s care facility.

Despite that, Campbell has performed on rare occasions and was well enough in January 2013 to record a song called “I’m Not Gonna Miss You” with producer Julian Raymond. According to Rolling Stone, the song was written by Glen Campbell: I’ll Be Me, a documentary highlighted by interviews with Paul McCartney, The Edge, Bruce Springsteen, Vince Gill, The Band Perry, Taylor Swift and Keith Urban.

The lyrics for the song are deeply personal. “I’m still here but yet I’m gone,” Campbell sings, notes The LA Times. “I don’t play guitar or sing my song/It never defined who I am/The man that loved you till the end/You’re the last person I will love/You’re the last face I will recall/And best of all…I’m not gonna miss you.”

“I’m Gonna Miss You” will be released online on Sept. 30 and will be included on Big Machine Records’ EP soundtrack for I’ll Be Me. The Band Perry also provided a recording of “Gentle On My Mind” for the album. Campbell’s daughter Ashley recorded “Home Again” for the release.

Live recordings of “Witchita Lineman” and “A Better Place” will also be on the EP.

I’ll Be Me opens in New York on Oct. 24.
